ニュージーランドの理学療法—現状と今後

はじめに

　ニュージーランドは南半球，オセアニア地区に位置する島国であり，2つの主要な島（北島，南島）をはじめとする大小さまざまな島々からなる．その面積は約27万km²であり^1），日本よりもやや小さい程度であるが（日本：約37万km²），人口は約420万人と^2），日本の約1億2,000万人よりもはるかに少ない．主要産業は羊などの畜産を中心とした農業であり，ラグビー強豪国として知られている．先住民族はポリネシア系のマオリ族であるが，人口の多くは英国をはじめとするヨーロッパ系民族であり，公用語は英語およびマオリ語である．英国連邦加盟国の一つであり，その文化および制度には英国と共通するものも多くみられる．

　ニュージーランドにおける理学療法の歴史は長く，理学療法先進国の一つと言える．1913年よりマッサージ師としてその教育が始まり，1920年に初めてその資格制度が法によって定められた^3）．その後，1949年より理学療法士として認められるようになり，現在までに約100年もの歴史をもつ．そのため，ニュージーランド社会における理学療法士の認知度は非常に高く，その活動範囲も広い．特に，理学療法士が自律性（開業権）を有しているのが一つの特徴である．2013年時点で登録されている理学療法士数は4,274名であり^4），日本の理学療法士数，約10万人と比べると非常に少ないが，人口比ではほぼ同程度である（理学療法士1人/約1,000人）．世界的に著名なニュージーランド出身の理学療法士として，Robyn McKenzieやBrian Mulligan，Stanley Parisなどが挙げられ，整形外科理学療法，特に徒手療法が発展している．

　そのような長い理学療法の歴史をもつニュージーランドであるが，日本社会同様に少子高齢化が進み，疾病構造が変化するにつれて理学療法士に求められる役割が少しずつ変わろうとしている．本稿では，まずニュージーランドの医療制度および理学療法の現状と今後の変化の見通しを解説し，日本人理学療法士の活動の展望について述べる．
