超音波エコーを用いた筋肉の評価と理学療法への応用

講座企画にあたって

 近年,超音波エコーは非侵襲的に身体内部の状態を把握することが可能なため理学療法分野でも使用の可能性が検討され,一部で臨床応用されはじめている.しかし,超音波エコーを医療現場で理学療法士が使用する場合には注意が必要である.医療現場では,どのような生体モニターや評価機器であっても,患者の身体に適用する以上,医師の管理下で医師の指示に従って行うことが求められる.

 また,理学療法士が心電図を装着したり,超音波エコーを当てたりすることは可能であるが,診断は不可というのが基本である.例えば,銭湯や薬局などに据え付けてある血圧計で,患者が自身で測定することは問題ないが,医師以外の誰かが「高血圧」と「診断」すると医師法違反になることと同様に,医療機関で超音波エコーを当て「診断」を患者に伝えた時点で,医師法違反になると考えられる.したがって,理学療法士が医療現場で超音波エコーを使用する場合は,その使用や結果の伝達については,細心の注意が必要となる.

 一方,医師や臨床検査技師が行った超音波エコーの結果を,理学療法士が正しく理解することは非常に有用で,超音波エコーの結果をどのように理学療法に活かしていくか,また,理学療法への応用をどのように浸透させ,拡大していくかは今後の課題でもある.
