重症下肢虚血患者に対する日常生活指導の実際

はじめに

　近年，末梢動脈疾患（peripheral arterial disease：PAD）の疾病構造は大きく変化し，バージャー病が減少する一方で，高齢化，食生活の欧米化などにより，糖尿病や慢性腎不全（維持透析）を有する患者が増え，その結果，閉塞性動脈硬化症が増加している．PADから重症下肢虚血（critical limb ischemia：CLI）に至る症例は決して多くないといわれているが，その絶対数は年々増えており，現行の治療法に抵抗性のCLIも多く，「再発・増悪」，「切断・再切断」，「心血管死」の割合が高い．

　CLIの予後は極めて不良であり，救肢と生命予後改善のためには，禁煙，内服遵守，食事療法，運動療法などによる下肢循環を悪化させるリスクファクターの是正が欠かせない．本稿では，CLI患者に対する日常生活指導の実際について解説する．
