症例報告

右開胸低侵襲心臓手術後に再膨張性肺水腫を発症し重症化した症例の理学療法経験

要旨　右開胸低侵襲心臓手術（minimally invasive cardiac surgery：MICS）後に再膨張性肺水腫（re-expansion pulmonary edema：RPE）を発症し，重症化した症例に対する理学療法を経験した．MICS後に合併したRPEの報告は非常に少なく，呼吸障害が長期化する例や拘束性換気障害が残存する例の報告はない．同様に，理学療法に関する報告例は見当たらない．MICS後にRPEを発症したが，これに対し術後早期から体位呼吸療法と日本循環器学会のガイドラインに則って，段階的離床，監視型運動療法を行った．その結果，退院時では呼吸障害の残存を認めたものの，独歩退院が可能な状態まで改善した．また，1年後の胸部X線で右肺の陰影の著明な改善が認められた．本症例の理学療法経験から，引き続き症例の蓄積を行い，MICS後にRPEを合併し呼吸障害が長期化する症例に対する理学療法の効果を明らかにすることが今後の課題であると考えられた．
