疼痛とニューロモジュレーション

はじめに

　難治性疼痛とは，その原因にかかわらず既存の除痛法では治癒させることが困難な疼痛の総称であり，その代表に神経障害性疼痛（neuropathic pain）がある．神経障害性疼痛とは，国際疼痛学会（International Association for the Study of Pain：IASP）では「Pain caused by a lesion or disease of the somatosensory nervous system」と定義されており，中枢神経および末梢神経系の体性感覚系の障害に起因するすべての痛みの総称である．実際にさまざまな原因による神経障害性疼痛が報告されている（表1）^1,2）．それらの多くはニューロモジュレーション療法の対象となり得る慢性の難治性疼痛である．

　2000年代後半より，神経障害性疼痛に対する薬物治療ガイドラインが欧米［IASP，European Federation of Neurological Society（EFNS）］および本邦（日本ペインクリニック学会）において策定され，煩雑であった神経障害性疼痛の薬物治療をevidence-basedに行うことが容易となった．しかしながら，ガイドラインに示された薬物の使用によっても改善が困難な神経障害性疼痛は多く存在し，そのような症例に対してさまざまなニューロモジュレーション治療が試みられている．

　本稿では，神経障害性疼痛に対する薬物治療の現状を解説するとともに，本講座のテーマである疼痛のニューロモジュレーション治療をその変遷を含めて解説する．
