関節リウマチ

関節リウマチ（rheumatoid arthritis：RA）は結合組織に炎症を来す全身性疾患です．関節滑膜に炎症が生じ，炎症性肉芽である関節パンヌスによって関節軟骨や骨が分解され，関節破壊が生じます1）．そのため関節変形，疼痛，腫脹，朝のこわばりなどが主な症状として生じます．ただしRAは全身性疾患なので，関節以外にも多様な症状を呈します．また薬剤が病態に及ぼす影響は大きく，歩行能力を評価する際には薬剤の使用状況の把握が重要です．メトトレキサートを代表とする疾患修飾性抗リウマチ薬（disease modifying anti-rheumatic drugs：DMARDs）や腫瘍壊死因子（tumor necrosis factor：TNF）阻害薬に代表される生物学的製剤は特に注意を要します．
