—理学療法に関するガイドラインupdate—脳梗塞急性期

はじめに

　厚生労働省による患者調査の結果では，脳血管疾患の総患者数は117万9000人とされ，減少傾向にあるとされている^1）．しかし脳血管疾患（脳卒中）は，介護が必要となった原因疾患の第2位（16.6％）とされており^2），社会復帰を阻害する原因となっている．そこで，脳血管疾患の後遺症に対するリハビリテーションの果たす役割は重要である．

　現在，臨床場面において脳卒中治療で利用されているエビデンスとして，『脳卒中治療ガイドライン2015』^3）および追補版2017がある．理学療法に関しては，2011年に『理学療法診療ガイドライン第1版〜脳卒中〜』が発行され，理学療法評価（指標）や介入の推奨グレードとエビデンスレベルなどが紹介されている．

　そこで本稿では，脳梗塞急性期の重症例を提示し，理学療法を進める際のリスクや疾患管理に関する医学的なエビデンスを，機能回復を促すための理学療法の臨床場面に応用する具体的な工夫と合わせて述べることとする．
