後外側腹側核を中心とした視床出血と理学療法

はじめに

　視床出血の好発部位として視床膝状体動脈や視床穿通枝動脈が報告されている^1）．視床損傷では，感覚障害が表現されることが多いように感じる．それは，体性感覚の中継核である視床の後外側腹側核（ventral posterior lateral nucleus；VPL核）の灌流域である視床膝状体動脈は視床出血の好発部位であり，その灌流域の出血により感覚障害を伴う症例が多いからではないかと考える．そして，視床出血＝感覚障害という病態のイメージが定着しているように感じる．しかし，視床損傷で生じる症状は感覚障害以外に多岐にわたる．

　本稿ではVPL核を中心とした視床出血の症例経験を通して，病態整理とそれに対する理学療法を解説する．
