皮膚温変化が治療経過を反映した複合性局所疼痛症候群の一例—患肢の無視様症状に着目して

要旨　一側上肢に生じた複合性局所疼痛症候群（complex regional pain syndrome：CRPS）に対し，理学療法介入を実施した結果，疼痛および自己身体への無視様症状が軽減し，疼痛と皮膚温との間に一定の関係がみられたので報告する．患側手の皮膚温が高く，患肢の無視様症状を呈したCRPS患者1名を対象に，理学療法介入として段階的運動イメージと関節可動域練習（range of motion exercise：ROM ex）を行い，評価として疼痛の強度をNumerical rating scale，手背皮膚温をサーモグラフィ，無視様症状は質問紙を用いて評価した．結果，段階的運動イメージ後とROM ex後で疼痛と患側手背皮膚温の変化が同様に推移した．さらに，6週間継続して介入した結果，疼痛と手背皮膚温患健差が減少し，4週経過時で患肢の無視様症状が消失した．客観的な生理指標である皮膚温を患者に提示することで，介入効果を患者本人が実感できた．さらに，CRPSの自己身体意識の改善に着目した介入の重要性が示唆された．
