Wallenberg症候群後のlateropulsionに対する運動療法とgalvanic vestibular stimulationの併用治療の効果—単一事例研究

要旨　【目的】Lateropulsion（LP）を認めた症例に対し，原因を前庭機能障害と推察し運動療法と直流前庭電気刺激（galvanic vestibular stimulation：GVS）を併用した介入の効果を検証した．【方法】40歳台，男性．研究デザインはBAB型シングルケースデザインを用いた．操作導入期はGVSと運動療法を併用し非導入期は運動療法のみとした．評価項目はScale of the assessment and rating of ataxia（SARA）と重心動揺計で測定した左右方向動揺平均中心変位，矩形面積，総軌跡長とした．【結果】SARAはB1期後に改善したが，それ以降の変化はなかった．左右方向動揺平均中心変位はA期後と比較してB1，2期後に著明な改善を認めた．矩形面積，総軌跡長は立位条件により変化が異なった．【結論】運動療法にGVSを併用することで前庭機能障害が原因と推察されるLPを改善できる可能性が示唆された．
