はじめに

 近年の急性期医療の進歩による救命率の改善と人口動態の高齢化に伴い,多疾患を重複した高齢慢性疾患患者が急増している.理学療法は運動療法が主な介入手段となり,最終的なゴールは健康関連生活の質(health related quality of life:HRQOL)の改善となる.運動療法の効果は,身体機能やHRQOLの改善など多く報告されているが,高齢患者に対しては運動療法のみの効果に限界があり,その阻害因子の1つには低栄養が挙げられる.したがって,栄養管理のもとで適切な運動療法を行うことは必須であるとも言える.本稿では,慢性疾患でみられる「悪液質(cachexia)」に焦点を当て,その病態背景から管理方法についてまとめる.
