前頭側頭型認知症発症初期からの症状変化とその対応

　前頭側頭型認知症（frontotemporal dementia：FTD）は，Pick球が出現しタウの異常蓄積があるタウオパチー，TDP-43の蓄積などによって大脳前半部の前頭葉や側頭葉の限局性脳萎縮を示す疾患群である．臨床症状と萎縮部位の違いから，3臨床病型；① 行動障害型FTD（behavioral variant FTD：bvFTD），② 進行性非流暢性失語（progressive non-fluent aphasia：PNFA），③ 意味性認知症（semantic dementia：SD）に大分される（図）^1）．FTDの頻度は認知症診断のなかで5％程度と低いが，障害を受ける部位が意欲や社会性などを司る前頭葉中心の障害のために，アルツハイマー型認知症やレビー小体型認知症よりもスタッフや介護者の負担感が大きい^2）．
