頸部振動刺激と半側空間無視

半側空間無視と自己中心参照枠

　半側空間無視（unilateral spatial neglect：USN）とは，病巣と反対側の刺激に反応しない，またそちらを向こうとしない症候で^1），脳卒中者の日常生活活動の自立を阻害する．USNの病態は複雑で，その病態の違いからいくつかに分類（サブタイプ）される^2）（図1）．患者は1つのタイプに分類されるわけではなく，複数のサブタイプを有している．どのサブタイプを有しているか判断することで，空間認知処理過程のうちどのレベルや領域が障害されるかを推察し，治療介入へと結びつける手がかりとなる^3）．

　空間領域の座標でUSNを分類すると，自己身体を中心座標として一方の視空間を認識しない自己中心空間無視と，固視した環境内にある物体を中心座標とした物体中心空間無視に分けられる^4）．自己中心空間無視では，眼球（網膜），頭部，体幹などの身体の位置関係に基づき，視覚情報・前庭感覚情報・頭部位置の固有受容感覚情報を統合して正中が規定され（自己中心参照枠），これを基準とした一方の視空間が無視される．
