脊椎除圧術・固定術

はじめに

　腰椎に対する脊椎除圧術や脊椎固定術は主に腰部脊柱管狭窄症や腰椎すべり症に適応される手術です．一般的に腰部脊柱管狭窄症や腰椎すべり症は，保存治療が優先されますが，保存治療による効果が乏しく日常生活などに支障を来す場合や膀胱直腸障害などの重度な神経症状がある場合に手術療法が選択されます．

　運動器疾患に対する手術を受けるすべての症例に共通することですが，手術に至るまでの経過は症例1人ひとりで異なります．また，多くの症例の主訴である痛みは目に見えません．そのため，リハビリテーションや臨床実習を進めるうえで重要になるのが症例との対話と考えます．各々の症例の訴えや客観的な評価をヒントに個別性のある病態解釈が重要です．そして，ガイドラインや個々の研究論文を参考にリハビリテーションを進めていく必要があります．
