股関節の低侵襲手術と理学療法

股関節の低侵襲手術の特徴

　進行期・末期変形性股関節症でADLやQOLに支障を来した場合，人工股関節全置換術（total hip arthroplasty：THA）の適応となる．本邦では長く後方進入法や側方進入法が主として用いられてきた．近年では筋間進入を用いた低侵襲手術が普及してきており，なかでも前方進入法（direct anterior approach：DAA）や前側方進入法（anterolateral approach：ALA）のような前方系進入法が主流となっている（図）．ALAには側臥位で行うorthopädische chirurgie münchen（OCM）と仰臥位で行うanterolateral-supine approach（ALS）がある．

　前方系進入法の特徴は筋や骨の切離を行わずに筋間を割くようにして関節内に到達することができる点にあり，従来の後側方進入法に比べると術後の機能回復が早く，脱臼率も低いことが報告されている^1,2）．またDAAとALSはともに仰臥位で手術を行うため，両側同時手術や人工膝関節全置換術（total knee arthroplasty：TKA）などの多関節同時手術を術中の体位変換を必要とせずに施行することも可能となる．
