人工膝関節置換術後の患者満足度向上に向けた取り組みの効果検証

要旨　【目的】本研究の目的は，人工膝関節置換術（knee joint replacement：KJR）後の患者満足度向上に向けた取り組みの効果検証を行うことである．【方法】あんしん病院で施行したKJR患者を対象とし，調査期間をPhase 0〜3に分類した．Phase 0では術前の患者教育，Phase 1では術後早期からの電気刺激療法を併用したトレーニング，Phase 2では術後2か月での不満足要因の聴取，入院中の患者教育，Phase 3では満足度低値症例の担当スタッフへのスタッフ教育を実施した．患者満足度は術後3か月，術後6か月に評価した．患者満足度の比較にはKruskal-Wallis検定およびSteel-Dwass検定を用いて検討した．【結果】Phase 0とPhase 3の患者満足度を比較した結果，術後3か月では80.3点から90.6点，術後6か月では83.5点から90.6点に向上していた（ともにp＜0.01）．【結論】KJR後の患者満足度向上には，膝機能や運動機能の改善だけでなく，個々の目標や心理状態に沿った患者ベースの取り組みも有効であることが示唆された．
