神経筋疾患における疼痛へのアプローチ

　国際疼痛学会では，痛みは「組織損傷が実際に起こったときあるいは起こりそうなときに付随する不快な感覚および情動体験，あるいはそれに似た不快な感覚および情動体験」と定義されている^1）．慢性疼痛とは「治療に要すると期待される時間の枠を超えて持続する痛み，あるいは進行性の非がん性疼痛に基づく痛み」と定義されている^2）．痛みの持続期間は慢性疼痛に定義されていないが，一般的に3か月以上を超えて症状が持続する病態を指していることが多い^3,4）．

　慢性疼痛は，心理的苦痛や機能障害と深く関連している．「疼痛の悪循環」を示す病態モデルとして知られる恐怖-回避モデル（fear-avoidance model）においては，恐怖，破局的思考，自己効力感の低下などの痛みに対するネガティブな反応に基づいて身体的・社会的活動を過度に回避した結果，痛みが慢性化することが説明されている^5,6）（図1）^7〜9）．また，この悪循環を断つことが慢性疼痛の改善につながると示されている^5,6）．
