縫工筋と内側広筋間における伏在神経膝蓋下枝の絞扼性神経障害が疑われた1症例—超音波診断装置を用いた病態解釈

要旨　［目的］伏在神経膝蓋下枝（infrapatellar branch：IPB）障害が起因となり，歩行時に膝関節前内側部痛を呈した症例を経験したため報告する．［症例紹介］症例は30歳台の男性である．縫工筋と内側広筋の間に著明な圧痛とTinel signが認められた．同部位に対して超音波診断装置（以下，エコー）による観察を実施した結果，両筋の滑走不全により，筋間を走行しているIPBに圧迫が生じていた．その一方，徒手的に両筋の滑走運動を促した状態で観察した結果，IPBの圧迫は生じなかった．［理学療法と結果］理学療法は縫工筋と内側広筋間の滑走性を向上させる目的で実施した．その結果，エコー所見では縫工筋と内側広筋間の滑走運動が確認され，IPBの圧迫が生じない様子とともに疼痛は消失した．［結論］IPBの走行次第では，縫工筋と内側広筋間における絞扼性神経障害を招く場合がある．その評価にはエコーが有効であり，圧迫ならびに疼痛が生じない条件を視覚的に検証することで，理学療法が確立し良好な結果が得られたと考える．
