鏡視下腱板修復術後における複合性局所疼痛症候群様症状の発症に影響する術前因子—決定木分析による検討

要旨　【研究の目的】複合性局所疼痛症候群（complex regional pain syndrome：CRPS）は鏡視下腱板修復術（arthroscopic rotator cuff repair：ARCR）後の合併症の1つである．本研究は，複数因子が影響するという仮説をもとに決定木分析を用い，発症予測因子を明らかにすることを目的とした．【方法】ARCRを施行した203例を対象とし，CRPS評価および術前肩関節機能評価として，関節可動域，等尺性筋力，疼痛，日本整形外科学会肩関節疾患治療成績判定基準（Japanese Orthopaedic Association Score：JOAスコア）を使用した．CRPS様症状を目的変数，単変量解析にて危険率5％未満の項目を説明変数とし，決定木分析を行った．【結果】CRPS発症率は23.1％であり，CRPS様症状発症の影響因子として術前JOAスコア，術前自動・他動外転可動域，術前他動結帯可動域，術前夜間時痛が選択された．最終的に6グループに分類でき，予測確率は0〜75.0％であった．【結論】術前に2ないし3項目の評価を組み合わせcutoff値に従い分類することで，より明確な発症予測が可能である．
