Anterior interval部の膝蓋下脂肪体に疼痛を呈した1症例

要旨　【目的】非外傷性要因により，anterior interval部痛が惹起された症例経験について報告する．【症例紹介】症例は60歳台の男性である．主訴は歩行時のanterior interval部痛であり，下肢伸展挙上テスト（straight leg raising test：SLR）にて再現痛が認められた．そこで，SLR時のanterior interval とその周辺部に対し，超音波診断装置を用いて観察した．SLRの角度が増大するに伴い，大腿骨顆部は前方に偏位した．また，膝蓋下脂肪体は表層かつ脛骨側に押し出され，anterior interval内で膨隆した．Anterior interval の遠位部には瘢痕様組織が観察された．【運動療法と結果】運動療法では瘢痕様組織の柔軟性を改善させたが，歩行時痛は著変しなかった．次いで，SLR角度の増大に伴う大腿骨顆部の前方偏位に対して，半膜様筋を中心に伸張性を改善させたところ，歩行時痛は消失した．【結論】Anterior interval 部痛の多くには瘢痕組織が関与している．しかし，本症例は大腿骨顆部の前方偏位といった非生理的な運動が，anterior interval に侵害刺激を加え歩行時痛が惹起されたと考えた．
