側頸部，側胸部痛に対して長胸神経の走行を意識した神経周辺組織への理学療法が有効であった車椅子バスケットボール選手の1症例

要旨　症例は車椅子バスケットボールチームに所属する男性であり，車椅子駆動期や練習後に，右側頸部から肩甲骨上部の疼痛を自覚した．疼痛部位を超音波ガイド下に確認すると，長胸神経の中斜角筋貫通部に強く疼痛を認め，支配筋である前鋸筋のなかでも上部線維を意識した肩45°屈曲位における肩甲骨外転筋の筋力低下を認めた．そこで，中斜角筋貫通部に対する徒手操作ならびに，上部線維の自動介助運動を実施した結果，疼痛や筋力低下は改善した．その後，競技に復帰したものの，第3肋骨高位を中心とした側胸部痛が顕在化した．疼痛部位を精査したところ，外側胸動脈と伴走する長胸神経に一致した．また，肩90°屈曲位における肩甲骨外転にて疼痛の再現が得られたため，外側胸動脈を指標に長胸神経に対する徒手操作にて局所的な疼痛を消失させたうえで，肩甲骨固定筋の収縮を反復した結果，側胸部痛は改善した．本症例を通して，長胸神経は側頸部，側胸部痛の一因となる可能性が考えられた．
