化膿性脊椎炎による慢性疼痛にて低活動が続いた不全対麻痺患者における理学療法プログラム修正後の活力と日常生活活動の改善：4年間追跡症例報告

要旨　対麻痺と腰痛を認め，長期の活動性低下を認めた化膿性脊椎炎患者に対し，慢性期において理学療法プログラム修正により活動量に劇的な改善を認めたため，活力とADLの変化について議論した．化膿性脊椎炎と診断された71歳の男性は，対麻痺と腰痛を認め，術後早期から理学療法が実施されたが，身体機能やADLの改善は認めず，術後10か月に介護老人保健施設に入所となった．腰痛による離床困難が続いたため，症例の意向の聴取から具体的に趣味を行うために必要な目標設定と治療プログラムの再構築を実施した．座位時間の延長，活力，ADLおよび活動量の改善，歩行練習への移行を認め，術後17か月に自宅退所となり，術後4年時も再発なくさらなるADL維持および向上を認めた．慢性期においても具体的な目標設定や治療方針の再構築にて，活力，ADLそして活動量の改善に有用と考える．
