はじめに
HIV感染症に対してHAART（highly active antiretroviral therapy）が導入されたことにより，HIV感染症の予後は劇的に改善し，日和見感染症の発症やAIDS（acquired immunodeficiency syndrome）による死亡は，少なくなった。だが免疫不全が進行した状態でHAARTを開始した際に，治療によって沈静化していた日和見感染症が悪化したり，新たに発症したりすることがある。これは，HAARTによりHIVのウイルス量が減少し，日和見感染症を起こす病原体などに対する免疫機能が急激に回復することによって，過度の炎症反応が引き起こされて出現する病態と考えられており，免疫再構築症候群（immune reconstitution inflammatory syndrome：IRIS，immune restoration disease）と呼ばれるようになった。HIV感染のない結核患者で治療開始時に病状が悪化する病態1）や，白血病に対する骨髄移植や，化学療法で白血球数の回復とともに炎症が再燃するこのような病態2，3）が知られているが，近年この症候群は主としてHIV感染症において用いられている。ここではHIV感染症で出現する免疫再構築症候群について，特に中枢神経合併症に主眼をおいて述べる。
