頸椎後縦靱帯骨化症に対する椎弓形成術後に水頭症を合併した1例

はじめに

　後縦靱帯骨化症に対する後方除圧術である椎弓形成術は確立された手術方法であるが，その合併症としてはC5麻痺や頸部の不安定性，感染症，人工骨離脱，などが挙げられる。しかし，椎弓形成術後に合併した水頭症の報告は過去に1例のみであり，非常に稀な病態といえる。本症例では，術中に硬膜損傷の所見がなかったにもかかわらず，術後1カ月間で水頭症を呈し，皮下髄液貯留がみられた。硬膜損傷とその後の皮下髄液貯留がなんらかの髄液の循環動態に変化をもたらし，水頭症をきたしたものと考えられるが，その詳細なる機序は不明である。本症例の臨床経過をふまえて考察する。
