特集 片頭痛の予防療法

はじめに
片頭痛は，日常臨床で遭遇することの多い疾患の1つであり，かつては致命的でないこともあり単なる持病として見過ごされることも多かった。しかし近年，片頭痛発作時（急性期）の治療薬として，トリプタン系薬剤（スマトリプタン，ゾルミトリプタン，エレトリプタン，リザトリプタン，ナラトリプタン）が使用されるようになり，片頭痛の治療は大きく変貌を遂げた。これらにより片頭痛患者の日常生活の活動性は著明に改善した。しかし，これらの薬剤が有効であっても高頻度に発作が出現する時には，鎮痛剤やトリプタン系薬剤の頻繁な使用により，さらに治療が困難である薬物乱用頭痛へと頭痛の内容が変容するなど，新たな臨床的な問題を起こす。このような事態とならぬように，予防薬として，発作とは別に連日服用する薬剤の使用が検討される。このようなときに使用される予防薬として，プロプラノロールなどのβ遮断剤，三環系抗うつ剤のアミトリプチリン，抗てんかん薬のバルプロ酸が既に知られている。最近，他の抗てんかん薬でも予防療法で有効性があるものがあることが知られつつある。本稿では，片頭痛予防効果のある（可能性のある）新しい抗てんかん薬につき，その臨床成績および作用機序，特に片頭痛の病態生理との関連について概説する。なお現在のところ，いずれの薬剤も「片頭痛」の病名では保険適応となっていないことも付記しておく。
