くも膜下出血急性期における中枢性低ナトリウム血症の早期診断―尿中ナトリウム排泄量測定の有用性

はじめに

　くも膜下出血急性期にみられる中枢性低ナトリウム血症（cerebral salt wasting syndrome，CSWS）は脳血管攣縮の発生に関与するとされている^1，2）。近年，atrial natriuretic peptide（ANP）やbrain natriuretic peptide（BNP）などのナトリウム利尿ペプチドがくも膜下出血（subarachinoid hemorrohage，SAH）術後に高値を示すことが報告され^3，4），このナトリウム利尿ペプチドの過多がCSWSの原因であるという報告が散見されるが，現在においてもその詳細な機序は明らかではない。一方，本病態に対する治療としては水分とナトリウムを十分量補うことが推奨されている^5）。また，ナトリウム利尿抑制と水貯留促進効果のある鉱質コルチコイドの有用性についても報告されているが^6，7），いかに早期にCSWSを予知し，選ばれた症例に対していかに早期に治療を開始するかの検討はなされていない。実際に治療開始時期が遅れたり，CSWSが重篤な症例では治療を行っているにもかかわらず低ナトリウム血症が遷延化してしまい，脳血管攣縮を増悪させてしまうことをしばしば経験する。今回われわれはくも膜下出血後の尿中ナトリウム排泄量に注目し，尿中ナトリウム排泄量がCSWSの簡便かつ有用な予測因子となり得るかについて検討を行った。
