一側の視床・線条体病変を認めた深部脳静脈血栓症の1例

　症　例　60歳，男性

　主　訴　頭痛，嘔吐，歩行障害

　既往歴　56歳時に上行結腸癌で右結腸切除術を受けたが，3カ月前に肺転移が確認された。

　現病歴　200X年5月7日，夕方に突然後頭部痛が出現した。翌朝になっても頭痛は持続していたが，そのまま外出した。外出先で嘔吐したため，直ちに帰宅し臥床した。正午に歩行障害を認め，呂律緩慢も加わったために同日16時に当科外来を受診した。

　入院時所見　頸部血管雑音は聴取せず，心音正常，心雑音もなかった。意識はJapan Coma Scale（JCS）3であったが，髄膜刺激徴候はなかった。脳神経系では麻痺性の構音障害を認めた。運動系では，Barre徴候で軽度回内，落下する程度の右片麻痺を認めたが，四肢腱反射は正常であった。感覚系と自律神経系とに異常はなかった。
