はじめに
痛みはAδ線維とC線維の2種類の求心性感覚線維によって伝達される。Aδ線維は薄い髄鞘を持ち，伝導速度は5～30m/秒である。この線維により鋭く，ちくりとした，局在が明瞭な痛みを感じる。C線維は無髄で伝導速度は0.5～2m/秒と遅い。この線維により，鈍く局在がはっきりしない，持続的な痛みを感じる1)（Table1）。
痛みは侵害受容器（nociceptor）が組織の損傷や組織損傷を起こす刺激に反応して生じる。侵害受容器の最も単純な形態は自由神経終末（free nerve ending）と呼ばれ，むき出しの無髄の終末が皮膚などの組織に終わっているものである。皮膚の侵害受容器には機械的侵害受容器とポリモーダル受容器がある。機械的侵害受容器は強い機械的刺激にのみ反応し，Aδ線維の終末に分布する（Table1）。一方，ポリモーダル受容器は強い機械刺激に加え，侵害性の熱刺激，化学的刺激，時には強い冷刺激にも反応し，皮膚ではC線維の神経終末に主に存在する（Table1）。
本稿では表皮の無髄線維の病態について解説するが，「表皮の無髄線維」とは上記の感覚線維，Aδ線維とC線維の神経終末を意味する。誤解のないように述べておくと，Aδ線維は細い有髄線維であるが，シュワン細胞に覆われるのは真皮までで，真皮と表皮の境界の基底膜を貫いてから先の表皮内では髄鞘がなく，無髄となる。量的には皮膚無髄線維はC線維が多くを占める。一方で，交感神経線維もC線維であり皮膚に豊富に存在するが，真皮内の器官（汗腺など）に分布し表皮には分布しないので，本稿では取り扱わない。
