ペインクリニックからみた慢性痛

はじめに

　筆者の所属は学際的なペインセンターではなく，麻酔科のペインクリニックである。関連する診療科とはさまざまに連携して，痛みを診療している。痛みの診断は，詳細な問診と神経学的所見の聴取に始まり，それと矛盾しない画像所見が確認できれば，治療方針は自ずと定まる。治療には薬物，神経ブロック，理学療法，心理療法，手術などがある。薬物治療は近年めざましく発達し，非癌性疼痛に麻薬性鎮痛薬の処方が可能となった。X線透視下や超音波ガイド下など安全，確実な神経ブロック手技が発達した。理学療法は専門部門に依頼するが，心理面の対応は手探りながら自前でも行っている。当科は脊椎疾患，帯状疱疹，関節痛，虚血肢などの急性痛も多いが，地域の医療機関で診断不明であるとか，治療に難渋している慢性痛が多数紹介される。

　本稿では，慢性痛に対するペインクリニックの取組みを紹介する。
