増大特集 痛みの神経学―末梢神経から脳まで

はじめに
痛みを感じるという現象は，一見単純に見えるが，実は相当に複雑な神経情報処理システムに基づいていることがわかってきており，他の感覚モダリティとは相当異なっている。例えば痛みの「中枢」が存在するかどうかという重要な問題についても，その解答はまだ明らかな形では得られていないこと（pain matrixの考え方も最近変化を遂げつつある1)）や情動との関連についても（パラレルな情報処理とシリアルな情報処理との関係性など）不明な点が多いことに注意が必要である。しかし，痛みの本体は本特集のテーマである末梢から脳までを総動員した，さらには自律神経反応などその先に含まれてくる出力系までによって表現されるものであることに間違いはないものと思われる。また，そこに痛みの慢性化が加わってくると，精神機能などの影響があり，もう一段問題は複雑になる。
そこで，本小論では痛みの情報処理における最も高次な過程であると思われる精神機能との関わりについて，慢性化した痛みの精神医学的問題を特に症候学的，診断学的視点から述べてみたい。
