はじめに

　パーキンソン病（Parkinson disease：PD）では錐体外路症状だけではなく，精神症状，嗅覚障害，自律神経症状などさまざまな非運動症状を認める。とりわけ自律神経症状については，便秘がPD発症前に出現する可能性¹⁾や心臓交感神経が早期に障害される²⁾ことが明らかになっている。これらの成績に加えて，Braakら³⁾のLewy小体の進展仮説によれば，PDの自律神経障害は末梢から中枢側へと病変が進展して行く可能性があり，非常に注目されている領域である。本稿ではPDの自律神経症状の特徴について，最近の知見を踏まえ概説する。
