人物誤認は妄想か錯覚か？

A　村井　カプグラ症状典型例にみる妄想

　精神神経科領域で人物誤認として分類されている症状は多岐にわたる。人物誤認とはそもそもどういう状態を指すのか，という表題のような問いを深く考えることなく，見かけ上なんとなく似通っている症状を次々に列挙していき，人物誤認やmisidentification syndrome（同定錯誤症候群）のリストを拡大していったためにそのような状況になったのではないかとも思う。したがって，表題の問い，すなわち「人物誤認は妄想か錯覚か？」についても，どのタイプの人物誤認（またはmisidentification）について考えるのかによって答えがまったく異なってくる。例えば，syndrome of subjective doubles（自己分身症候群）のように自己の重複化や替え玉をテーマとしたさまざまな症状も「人物誤認症候群リスト」¹⁾では，人物誤認の一種とみなされているが，自己を対象とした人物誤認の場合と，他者を対象とした人物誤認の場合では，表題の問いに対する議論はまったく異なってくる。したがってここでは他者に対する人物誤認に議論を絞り，さらにそのような人物誤認の代表として，報告者の名前を冠して知られるカプグラ症候群²⁾に照準を合わせて議論を進めていく。人物誤認（またはmisidentification）について包括的な議論を行っている研究者のほとんどが，今から1世紀近く前にカプグラ（Jean Marie Joseph Capgras；1873-1950）らによって報告された症例を範例とみなしたうえで人物誤認（またはmisidentification）全般について考察してきたからである。

　カプグラらによって報告された53歳の女性（M夫人）は，その精神医学的診断は今日でいえば統合失調症に該当するが，自分の周囲の多数の人物について，外見がそっくりの替え玉が存在すると訴えた。カプグラ自身は，この症候をillusion des 'sosies'（「ソジー」の錯覚）と呼んでいる。Sosieとはフランス語で「瓜二つの替え玉」という意味を持つ。
