CLIPPERS—chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids

文献概要

CLIPPERSは，2010年にPittockらによって提唱された新しい疾患概念である。病理，画像，臨床的特徴を名前に盛り込んだ疾患名で，すなわち，chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroidsの頭文字よりなる。特徴的な画像所見は，橋を中心として造影後，点状および弧状の造影効果がみられることである。再発時に橋を中心とした病変とみられることも重要な所見となる。本疾患に特異的なバイオマーカーはないので，常に他の疾患との鑑別に留意する必要がある。種々の鑑別の中で，いわゆる前リンパ腫の状態である，GradeⅠリンパ腫様肉芽腫症や悪性リンパ腫のsentinel lesionとの鑑別が困難であるが重要となる。
