ALS皮質運動神経興奮性と神経細胞死—Dying Forward仮説

シャルコーは19世紀後半に，筋萎縮性側索硬化症（ALS）の病態進展を上位運動ニューロン障害から始まり下位運動ニューロンへと拡がると推測した。それから約150年の歳月が流れたが，いまだこの疑問に対する答えは出ていない。一方，ALS運動神経細胞死の原因の1つとして，神経興奮毒性が近年注目されている。この総説では，ALSにおいて報告されている皮質運動神経興奮性と運動神経細胞死の関係について概説し，ALSの病態進展について考察する。
