文献概要

神経難病の中でも亜急性に進行する疾患においては，患者は時として疾患を受容できないまま人生のいろいろな選択を迫られる。そのため医師には早期に正確な診断をすることが求められる。しかし臨床の場では，疾患の全体像の中の限局した病歴，症状のみから診断を求められることがしばしばある。さらに，解釈に悩む診察・検査所見が多数提示され，その組合せによっては診断基準やガイドラインの項目を満たしてしまい，結果的に机上で疾患を成立させてしまうこともある。今回1つの例として，当科に受診した症例を参考にそれら「悩ましい」所見を挙げ，ピットフォールに陥らないよういま一度意識することを促したい。
