てんかんの診断・分類—てんかん診療における思考過程

てんかんは発作を繰り返す慢性疾患であり，大脳の神経細胞が過剰に興奮するために発作性の症状が生じると定義される。その病因（原因）はさまざまで，多彩な併存症を有する。本論では，てんかんの診断と分類，てんかん症候群についてILAEの診断基準に基づいて解説する。また，具体例を挙げて，てんかんの診断プロセスについて解説する。てんかん発作では脳機能の一過性の変容が生じており，その診断や焦点推定においては脳機能局在やてんかん性活動の伝播様式などの知識に基づいて病歴聴取を行い，発作症候を解釈することが重要である。てんかん診療に際して，薬物や外科治療のみならず，てんかんの正確な診断においても高い診療能力・問診力が求められる。
