小児期に発症し，著しい脳萎縮を呈した42歳男性

〔現病歴〕1歳6カ月時麻疹に罹患したが重症化せず自然治癒した。7歳頃まで正常に発達したが，8歳頃より語尾の不明瞭化，食事をこぼす，わずかな段差で転ぶといった退行が出現し，脈絡のないことを話し表情が乏しくなったため小児科を受診。脳波および髄液所見から亜急性硬化性全脳炎と診断された。イソプリノシンを内服開始し，プロチレリン（TRH）の静注，インターロイキン-2，ピシバニールによる治療が行われたが排泄介助，経口摂取困難，会話不能，寝たきりとなりミオクローヌスも出現した。

　8歳7カ月，当院に転院した。インターフェロンおよび細胞性免疫賦活薬を投与しいったん改善がみられたが徐々に増悪し，9歳頃より全身性痙攣が出現した。13歳時，頭部MRIで大脳／脳幹の全体的な萎縮を認めた（Fig. 1A）。
