Case
動悸・息切れを主訴に受診し，肺塞栓症の診断に至った症例
患者：20代，女性．
主訴：動悸，息切れ．
病歴：2週間ほど前より，労作時に動悸・息切れが出現するようになった．
失神や胸痛はなし．いつものパニック発作とは違うが，動悸が出現するとパニック状態になることもある．増悪傾向のため受診．
既往歴・内服歴：パニック障害にて抗不安薬数種類内服中，月経困難症にてピルを内服中．
身体所見：SpO₂ 96％，呼吸数20回/分，心拍数104回/分，血圧100/55mmHg，体温36.8℃，呼吸音清・心雑音なし，甲状腺腫大なし，眼球結膜貧血なし，下腿浮腫なし．
検査：採血・心電図・X線は異常なし．
経過：ストレスも多く精神的なものであろうとのことで経過観察に．翌日胸痛・ショック状態で救急搬送され，造影CTにて肺塞栓症が判明，ICU入院となった．
