　高齢者人口が増加し，平均余命が延長していくことは同時に，「複数の慢性疾患をもつ人口」の増加につながることは容易に予想できる．特にプライマリ・ケア現場においては，こうした人たちのケアをどのように組織化していくか，ということが重要な課題として認識されている．
