8．NT-proBNPが異常値だったら

Case
腎血管性高血圧症による急性心不全の1例
患者：87歳，女性．　　既往歴：高血圧
現病歴：夜間入眠中に突然の呼吸困難を訴え，当院へ救急搬送となった．起座呼吸を呈し，血圧242/140mmHg，心拍数120回/分，呼吸数32回/分，SpO₂ 88％（酸素10l/分投与下・リザーバーマスク）であった．酸素投与下の血液動脈ガス分析では呼吸性アシデミアを認め，採血検査では心筋逸脱酵素の上昇は認めなかったものの，BNP 1,024.5pg/ml，NT-proBNP 13,674pg/mlと著明な上昇を認めた．胸部X線ではbutterfly shadowを認めたが，心電図でも心エコーでも虚血性変化は認めなかった．
経過：クリニカルシナリオ1^1）の急性心不全と診断し，血管拡張薬を用いて降圧を開始，さらに非侵襲的陽圧換気（NIPPV）を用いて呼吸状態を改善させた．各種精査の結果から右腎動脈狭窄を認め，カテーテル治療を行って血圧は安定化した．BNP 58.7pg/ml，NT-proBNP 784pg/mlまで改善し，独歩退院となった．
