あちこち痛い

Case : 診断は何か？
患者：36歳，男性．
主訴：1年前からあちこち痛む．
現病歴：1年前から，右足関節の腫脹と疼痛があり，その後左足関節，両膝，股関節，腰背部にも痛みが拡がってきた．腰背部痛は徐々に出現しており，安静時に強く，身体を動かしているとやや緩和する．
　2カ月前に近くの整形外科を受診したところ，X線では「特に骨に異常はない」といわれ，ロキソニン^Ⓡ，ミオナール^Ⓡ，ムコスタ^Ⓡを処方された．症状は少し改善したが，また増悪し，疼痛のため歩行が困難となり，臥位になるのもつらくなってきたので心配になり，当科外来を受診した．
　数カ月前から右の踵やアキレス腱も痛くなり，特に起床時に足をつくと踵が痛かった．右第3指や左第4趾ごと腫れ上がって痛んだこともある．
