本当に頸椎症ですか？—頸椎症性脊髄症の診断とピットフォール

Case
患者：65歳，女性．
現病歴：1年前から両手が使いにくくなり，X線写真にて頸椎症と診断されている．また同時期より疲れやすくなり，あまり出歩かなくなった．ちょっとしたことで怒るようになり，心療内科にも通院し，向精神薬の処方を受けているが，気持ちの落ち込みは改善せず，1年間で5kg痩せた．両上肢の筋力低下としびれが増悪してきているため受診．
身体所見：腱反射は左右差なく，上肢で低下し，下肢は亢進．Hoffmann反射は両側で陽性．他覚的な感覚障害はない．他院で施行した頸椎MRI読影所見では，C4/5，C5/6で，脊柱管狭窄を認めている．
