Case
定年退職を迎えたコントロール不良の2型糖尿病の一例
患者：70歳，男性．2型糖尿病で定期通院中．妻と2人暮らし．
所見：BMI 27kg/m²．随時血糖160mg/dl，HbA1c 8.2％．
現病歴：運送会社に勤務していたが，60歳で定年退職している．食事は専業主婦である妻がつくったものを食べているが，間食が多いため，これまでHbA1cは9％程度で推移しており，良好とは言えない．
　本日は，いつもと違い妻同伴で来院した．妻からは「最近何を聞いても答えが曖昧で，認知症が心配なのだが大丈夫だろうか」と質問を受けた．
処方：メトホルミン1,000mg分2，1日1回のインスリン療法（インスリングラルギン 20単位）．
合併症：増殖前網膜症，糖尿病腎症3B期．糖尿病神経障害なし．
