よくある症状が「薬剤の副作用」だったなんて！—“いつもの処方”のピットフォール

あるあるナゾ処方Case
ビスホスホネート製剤による食道炎から漏出液を伴う両側下腿浮腫をきたした一例
患者：74歳，女性．
主訴：食思不振，両側下腿浮腫，右下腿の難治性潰瘍．
現病歴：もともと入院の3カ月前から徐々に食事が進まなくなってきていたが，何とか食べていた．徐々に下腿がむくむようになり，足も重くなってきたとのこと．
　入院の4日前に階段を踏み外して右下腿を打撲し，徐々に潰瘍化した．全身状態の緩徐な悪化と潰瘍の加療目的で，近医より紹介となり入院となった．
既往歴：左大腿骨骨幹部骨折（73歳時），胃癌（54歳時），骨粗鬆症．
内服薬：リセドロン酸ナトリウム2.5mg/日（ビスホスホネート），カモスタットメシル酸塩300mg/日．
入院時現症：身長148cm・体重35.5kg，BMI 16.2．バイタルサイン安定．
　両側下腿に著明な浮腫（fast pitting edema,漏出液がしみ出している），右下腿に5×7cmの潰瘍あり．
検査所見：Alb 1.9g/dl，BUN 20mg/dl，Cr 0.61
mg/dl．BNP 36.8pg/ml．TSH 3.520μIU/ml，FT4 1.0ng/dl（正常下限），FT3 1.00pg/ml（低値）．VitB₁ 41ng/ml（正常）．
入院後経過：上記より，栄養不良による低アルブミン血症・浮腫・創傷治癒の遷延をきたしていると考え，食思不振の改善を目指した．来院時に随伴症状を聞くと，胸焼け，胸骨裏の不快感があり，食道炎を考えリセドロン酸ナトリウムを休薬してスクラルファートを投与したところ，入院4日目には安定して食事摂取が可能となり，徐々に血清アルブミン値も改善し，浮腫・潰瘍も軽快して退院となった．
　上部消化管内視鏡は本人に抵抗感があり未施行ではあるが，るいそう・亀背が強く，ビスホスホネート製剤による薬剤性食道炎が起こりやすい素因があった．食道炎により食思不振が起こり，更にるいそうになるという悪循環が強く疑われる症例であった．
