　日常診療において指針になるのは，エビデンスばかりではありません．臨床経験に基づく個別の知恵や直観もまた，おおいに頼りになるもの．そこで，日々の実践に基づく工夫や考察，経験知を，言語化して共有する新コーナー「Empirical EYE」を始めます．
　今回は，プライマリ・ケア認定薬剤師として，“クリニック薬剤師”という稀少な立場から「処方」に関わり，患者そして医師をはじめとする多職種とのコミュニケーションに基づいて実践してきた「ポリファーマシー対策」の経験則をご報告いただきます．（編集室）
