悩ましい外側（水平）半規管型BPPV（背地性眼振タイプ）

Case
外側（水平）半規管型BPPVクプラ結石症をHead tilt hopping法で治療
患者：64歳、女性。
現病歴： 2年前より寝返りで、3カ月前より上下方を向くとめまい感が出現してきたので来院した。難聴や耳鳴りはなく、複視や構音困難、顔面のしびれなどの脳神経症状もなかった。
臥位頭位眼振検査（図1）：めまいを伴う方向交代性背地性（上向性）眼振が潜時なく2分以上持続し、強さは右より左耳下頭位で優位であった。また、仰臥位正面では右向き水平性眼振が認められ、右下約20°の頭位で消失した。
診断：右の外側（水平）半規管型クプラ結石症。
治療：Head tilt hopping（Yamanakaホッピング）法を施行した。診察室で左右頭位傾斜で10回ずつのホッピングを3セット行った。直後より、方向交代性背地性眼振は向地性に変化した。クプラから耳石粒/塊が剥がれて半規管脚に浮遊していると考え、頭位治療（Half Roll法）を行ったところ、眼振とめまいは完全に消失した。
