Case
患者：不明熱の39歳、女性。
現病歴：X月Y日、37.8℃の発熱に気づく。Y＋3日後、近医受診し、上気道炎疑いでアセトアミノフェンの投与を受ける。アセトアミノフェンを服用してしばらくは解熱するが、すぐ38℃台に発熱し、改善が見られないため、Y＋7日後、当院救急外来を受診。当直医がバイタルサイン、採血などをみて緊急性がないと判断し、ロキソニン^®を処方して帰宅となる。しかし発熱とふしぶしの痛みが続くため、Y＋12日後、当院総合内科を受診する。
　診察では、バイタルサインなど異常なし。体温37.6℃。全身リンパ節腫脹なし。皮疹なし。心音・呼吸音に異常なし。両手関節に熱感と腫脹、他動痛と関節腔の圧痛を認める。同様の関節炎を示唆する所見が両肘関節、両肩関節に認められた。胸部X線異常なし。一般採血では、末梢血白血球数異常なし。白血球分画異常なし。ESR（赤沈）67mm/hr、CRP（C反応性蛋白）4.5mg/dL、その 他の生化学的検査異常なし。発熱と急性多関節炎として精査を開始した。
　まず採血にて抗核抗体、RF（リウマチ因子）、抗CCP抗体などを外注でオーダーした。1週間後（Y＋19日）の外来でそれらが陰性、SLE（全身性エリテマトーデス）なども陰性であることを確認した。前回の外来以後、38℃を超える発熱は連日続き、関節炎症状もあるため、皮膚色、熱型、脈拍などの確認、血液培養採取のため入院を提案したが、家庭の都合で断られた。行き詰まった主治医は、抗RNP抗体、P-ANCA（抗好中球細胞質抗体）、C-ANCA、クリオグロブリンなど、陽性であれば確定診断に近づく、特異度の高い検査を含めたあらゆる免疫生化学検査を、絨毯爆撃的に外注でオーダーした。
　その1週間後（Y＋26日）、それら特異的検査はすべて陰性であった。困り果てて上級医に相談したところ、偶然、貧血があったために、何も考えずにオーダーした、“ある検査”が異常高値であることを指摘された。患者に皮膚の色調の変化の有無を改めて確認したところ、「発熱時に皮膚がピンク色になるような気がする、でもお風呂に入ったらそんなもんではないですか」と答えられた。そこで、ある診断に辿り着いた。患者は診断の遅れに怒りを見せながらも、治療を開始することには同意してくださった。
