入院中に下腿痛を訴えた高齢女性

セッティング
あなたは300床の総合病院（郊外）の総合診療医。各科専門医が常勤で少人数ずついるが、総合診療科が内科系入院を幅広く担当している。
Case
山川さん（仮名）というアルツハイマー型認知症で施設入所中の79歳女性が、来院前日からの悪寒・食欲低下・37℃台の微熱のため施設職員に連れられ外来受診し、「尿路感染症」の診断で入院治療を開始された。入院4日目の朝、あなたが回診に行くと、山川さんが左下腿の痛みを訴えている。バイタルサインは血圧152/86mmHg、脈拍数84回/分・整、呼吸数16回/分、体温37.2℃、SpO₂ 96％（室内気）。頸静脈怒張なし、呼吸音・心音に特に異常を認めない。 左下腿に腫脹と把握痛を認め、あなたは「DVT（下肢静脈血栓症）」を疑った。
既往歴：関節リウマチ、アルツハイマー型認知症、骨粗鬆症。
手術歴：特になし。
常用薬：プレドニゾロン10mg分2、セレコキシブ200mg分2、オメプラゾール10mg分1、アルファカルシドール0.25μg分1
社会歴：元飲食店経営。夫は肝癌で逝去。施設近隣に長女家族が在住。喫煙歴・飲酒歴なし。
入院前ADL：室内は歩行器使用で歩行可能。更衣・食事摂取・排泄は自立。
