急性虫垂炎を疑った時

はじめに
急性虫垂炎の超音波診断は確立されている？
　急性虫垂炎の超音波診断は以前から実施されていますが、現在もさまざまな観点で臨床研究が行われています。小児では画像診断の第一選択としてコンセンサスが得られていると思われますが、成人では超音波の位置づけは施設によって異なるようです。急性虫垂炎は頻度の高い疾患で、多くは典型的な所見を呈しますが、なかには診断が難しい場合も少なくありません。見逃しが許されない急性虫垂炎を、検者依存性の高い超音波ではなく、客観的なCTで確認したいという思いは、臨床医として自然なことでしょう。しかし、特に小児や女性では放射線被ばくを考慮し、超音波を優先すべきと考えられます^1）。痩せた成人でも虫垂の描出はそれほど難しくなく、積極的な利用が望まれます^1）。
　上記は専門家が行う超音波の話ですが、近年Point-of-Care超音波においても、急性虫垂炎が注目されるようになってきました。年齢を問わず頻度が高く、見逃しが許されない疾患であり、身体所見をガイドにして超音波の観察範囲を右下腹部に絞り込むことができるからでしょう。小児領域では、トレーニングを積んだ（小児）救急医によるPoint-of-Care超音波の有用性が近年報告されています^2）。成人でもケースによっては、Point-of-Care超音波で診療の質向上が期待できます^3）。
