頭痛患者で頭が痛い—ちょっと変わった一次性頭痛たち

　頭痛診療において病歴のOPQRSTは大切です。たとえば「Onset（発症様式）」が突発であれば、くも膜下出血など重大な疾患を除外しなければなりません。片頭痛を示唆するとされる「POUNDing^1）：Pulsatile quality（拍動性）、duration 4-72hOur（4〜72時間続く）、Unilateral location（片側性）、Nausea/vomiting（嘔気・嘔吐）、Disabling intensity（日常生活に支障あり）」も、OPQRSTの1つの亜型と言えます。一次性頭痛と言えば、片頭痛と緊張型頭痛が圧倒的に多いのですが、本稿では、それ以外の一次性頭痛の診断のポイントについて解説します。
